Billing

 Patients with Insurance Coverage

Please bring your insurance card to every appointment -Please settle your financial obligations in a timely manner
Provide updates to our office regarding changes in your address, phone numbers or insurance coverage
Copays and outstanding balances are due at your appointment. We are required to collect your co-payment at the time of service by your insurance company. If you are not able to pay your co-pay at the time of visit you may reschedule your appointment. If you still wish to be seen, there will be an additional $20.00 charge to cover the increased cost of billing.
Please contact your insurance carrier prior to your scheduled appointment to verify that our physicians are covered under your plan.

Claims will be prepared and submitted to your carrier provided we have all the necessary information. Co-Pays, deductibles and any services not paid or covered in full by the insurance are the responsibility of the patient. We will be happy to assist you with any questions concerning your particular coverage, our fees, or to set up payment arrangements. We currently participate with Medicare and many group insurance plans. Unfortunately, we do not accept Medicaid as a primary insurance.

Self-Pay Patients

All payments are due in full at the time of the visit.
We can give you an estimate of charges prior to your appointment. This will be an estimate only. The exact charge will be determined based on the professional and associated services provided during the appointment.
We currently accept Visa, MasterCard and personal checks as forms of payment. A $20.00 fee will be assessed on all returned checks.

Collection of Payment

Charges greater than 90-days past due will be subject to collections. We will exhaust every effort here before we ask for outside help. If you have an outstanding claim, please call us so an arrangement can be made. Sending you to a collection agency is the last thing we want to do! If your account is in collections you will be required to pay your account balance in full prior to scheduling any future appointments.

Worker Compensation

Our practice does NOT handle Worker Compensation claims. It is recommended that you contact your human resources department for assistance in locating a Worker Compensation affiliated physician.

Motor Vehicle Accidents:

Our practice will gladly see you for Motor Vehicle Accidents; however a payment of $150.00 is due prior to being seen. We will submit the claim to the insurance carrier on file. You will be refunded your money once your insurance company pays the claim. If the claim is not covered by your medical insurance, the patient can request a claim form to submit for reimbursement from the responsible auto insurance company.

Medical Record Copying Fees

There is a fee for copying and mailing medical records. This charge is the allowable amount per guidelines set from the State of Ohio. If you have questions regarding the fees, please call the billing office. The fee must be paid before we will release records.
