We’re working to improve your care
We’re committed to providing you with high quality care that meets your unique needs. By partnering with Medicare and other health insurance plans in the CPC Initiative, we hope to work together with you to coordinate your care visits, and create a care plan that’s right for you
Under the CPC Initiative, all of these insurers will give our practice additional resources to improve the care we offer to you and other patients. These resources will allow us to make investments that can help our practice better coordinate your care.
We won’t be charging you any extra fees because of these new services, and you’ll benefit from more coordinated care.
You can still use any doctor or hospital
You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your Medicare benefits aren’t changing, and this isn’t a Medicare Advantage plan or HMO plan. We may continue to recommend that you see particular doctors for your specific health needs, but it’s always your choice about what doctors you use or hospital you visit.
The CPC Initiative is something we participate in with Medicare, so there’s nothing you need to do to continue getting care from us.
Your health information from Medicare helps us give you better care
Our goal is to give you the right care, in the right place, at the right time. Soon, Medicare will start sharing some of your personal health information with us. This information may include things like dates and times you visited other doctors or a hospital. With this information, we’ll have more up-to-date information about your health.
Your privacy is very important to us. Just like Medicare, we put important safeguards in place to make sure all your personal health information is safe.
You can choose not to share personal health information about care you received
If you want Medicare to share this personal health information with us, you don’t need to do anything else.
If you want to stop Medicare from sharing this information, you should take one of the following steps:
- Call 1-800-MEDICARE (1-800-633-4227) and ask about “Comprehensive Primary Care.” TYY users should call 1-877-486-2048.
- Complete and sign the “Declining to Share Personal Health Information” form available in our office.
If you decide not to share your information now, but change your mind later, you can always update your preferences.
