New Patient Resources


In order for us to best service your medical needs, it would be extremely helpful for us to have access to your past medical records. It will enable us to have accurate records of any medical conditions you may have, immunizations you have received, and specifics on any past surgeries, hospitalizations, or medication history.

If your previous medical care was through the Cleveland Clinic, or the physician used EPIC for their medical record documentation system, we should be able to have access to your medical records. Therefore, you will not need to do anything special to allow us access.

If your previous medical care was anywhere other than through the Cleveland Clinic system, we ask that you request a copy of your medical records from your previous physician be sent to us. They will require that the request be put in writing.

A “Request to Release Patient Information” is included in your new patient packet, or you may download the release of records form, located under the FORMS tab.

You may either send it to them directly or we will be happy to forward it to them on your behalf.  (Under Ohio Revised Code 3701.741, they are legally allowed to charge you a standard fee for this service.)
